Keep a useful daily record
A daily record is a personal account of what you noticed. It can help you bring examples to a conversation, without asking you to interpret a symptom or make a diagnosis.
- Write the date or approximate time, then describe what you noticed in your own words.
- Add context you remember, such as what you were doing. Separate what you observed from what you are unsure about.
- Note a question you would like to discuss. This sheet does not decide whether a symptom is serious or what caused it.
These are general organization guides, not clinical instructions. Local health services and your health professional can advise on care.

Your worksheet
Fill in this sheet on screen, or print a blank copy to complete by hand. Nothing you type is sent or saved by this website. Your entries clear when you leave, hide this tab, change language or choose Clear. Keep this page visible while you use it.
Printing creates a paper or printer copy. Saving as PDF leaves a file containing your entries on your device. We do not receive or store your entries. The page clears after the print dialog closes, including if you cancel; it cannot delete your paper, PDF or printer copies. Keep those private.
