Prepare for an appointment

You decide what matters to you. A short list can give the conversation a starting point and help you remember the questions you wanted to ask.

  1. Put your main reason for the appointment first, then list the questions you most want to discuss.
  2. Gather relevant records and a current list of medications and supplements. Include allergies you need the professional to know about.
  3. Leave room to write down what you agreed on, what happens next and whom to contact with follow-up questions.

These are general organization guides, not clinical instructions. Local health services and your health professional can advise on care.
