Prepare for an appointment
You decide what matters to you. A short list can give the conversation a starting point and help you remember the questions you wanted to ask.
- Put your main reason for the appointment first, then list the questions you most want to discuss.
- Gather relevant records and a current list of medicines and supplements. Include allergies you need the professional to know about.
- Leave room to write down what you agreed on, what happens next and whom to contact with follow-up questions.
These are general guides for organising information, not clinical instructions. Local health services and your health professional can advise on care.

Your worksheet
Fill in this sheet on screen, or print a blank copy to complete by hand. Nothing you type is sent or saved by this website. Your entries are cleared when you leave, hide this tab, change the language or choose Clear. Keep this page visible while using it.
Printing creates a paper or printer copy. Saving as PDF leaves a file containing your entries on your device. We do not receive or store your entries. The page clears after the print dialog closes, including if you cancel; it cannot delete your paper, PDF or printer copies. Keep those private.
