Bring a clearer medication list

This is a list-making aid, not a dosing tool. Use it to bring information together for a conversation with your doctor or pharmacist.

  1. Copy each medication’s name and instructions from its current label. Include non-prescription medications and supplements you want to discuss.
  2. Mark anything you are unsure about as a question. Do not guess missing details or use this sheet to change a dose or schedule.
  3. Check the list with your doctor or pharmacist. Update your own copy when they confirm a change, and store it privately.

These are general organization guides, not clinical instructions. Local health services and your health professional can advise on care.
